COVID-19 Testing Consent Information and Forms
There are links to the 22-23 consent form for your child to be Covid-19 rapid tested here at Central High School, if they display symptoms of Covid-19 while they are on campus. This is a voluntary testing program and will need to be approved by you. Please select one of the below options to complete the form if you want your child to participate.
