Free Athletic Physicals

Click below for the packet for the AHO FREE Physical Event taking place on Sunday, June 10th at the Wells Fargo Center.    The deadline for these packets to be returned to the Athletic Office Room 63 (completely filled out), will be Friday, May 18th.  Anyone attending this event MUST fill out this packet completely in order to be screened (incomplete packets will be turned away).  In addition, this packet MAY NOT be used for any other physical examinations anywhere outside the AHO Event.  For students obtaining their own physicals, the general PIAA CIPPE Form must be used, which can be found on our website.

*Please note that this packets contain all necessary waivers, as well as Sections 1-5 of the PIAA CIPPE Form.  Students who attend the event will receive Section 6 for the physical process upon arrival.
